Severe — mostly horizontal
You start lying down. Ankle pumps, breathing, a two-minute seated attempt. Your nervous system learns that movement is safe before upright is safe.
Moderate — functional but crashing
You break the boom and bust cycle. Short movement blocks, mandatory rest after, strict pacing rules. You stop before you want to. Every session.
Mild — moving but still symptomatic
You shift to recumbent-first exercise. Rowing, recumbent bike, swimming. You build a base before attempting upright cardio. HR is information, not a verdict.
